MISSION:
To elevate the health status of American Indian/Alaskan Native people in our service area to the highest level possible through a comprehensive system of preventive and therapeutic services. To provide a broad range of culturally acceptable personal and public health services which will assist us to reach our goals of health and longevity for the people we serve.
SERVICES AVAILABLE:
- Out-patient Direct Medical and Specialty Care: Out-patient Direct Care refers to any and all services provided within the facility of the Fort Mojave Indian Health Center (FMIHC), either by our full-time providers or specialty providers who practice occasionally. These services are available to anyone eligible to be treated at FMIHC. These services include:
- Family Practice
- Pediatrics
- Podiatry
- Optometry
- Ophthalmology
- Dental
- Women’s Health
- Diabetes Management
- Behavioral Health (see department specific resource form)
- Ancillary Services: Ancillary Services refers to additional patient care services that support providers within the facility, and are available to American Indian/Alaskan Native patients eligible to be treated at FMIHC. These services include:
- Pharmacy
- Laboratory
- Immunizations
- Nutrition Counseling
- Pediculosis Eradication
- Public Health Nursing
- Prenatal Education
- Referral Contract Health Services: Contract Health Services (CHS) is a source of funding used to pay for specialty care services not available at FMIHC and can only be tapped for people who are compliant, eligible patients. Strict eligibility criteria exists and, due to the complexity of requirements, should be determined by knowledgeable employees of FMIHC
- Types of covered referred patient care varies, is designated to a priority level, and is dependent on demand and availability of funding
- Patient and Community Support Services: FMIHC offers additional support and care to Tribal Members of the Fort Mojave Indian Tribe. These services include:
- Respite Care
- Durable Medical Equipment and Supplies
- FMIHC extends additional support and care to eligible American Indian/Alaskan Native people who reside on or near the Fort Mojave Indian Reservation:
- Transportation
- Community Health Nursing
- Community Health Representatives
- Community Outreach for Education
- Health & Fitness Activities
- Health Promotion & Disease Prevention
- Patient Benefits Coordination
- Talking Circles with the Elders
ELIGIBILITY CRITERIA:
Patient must provide proof of Tribal Enrollment or Descendency in a Federally Recognized Tribe
